However, PPO and Indemnity insurance plans normally base the coverages provided on general dentist cost, and pay out coverages based up to the UCR (Usual, Customary & Reasonable) cost of a general or family dentist. Since specialist will normally cost more then a family or general dentist, it is important to understand you may have applicable UCR fees.
